Instructions for Requesting Scholarship Funds

Your scholarship award funds will be sent directly to the college, university or technical institute that you are attending in the fall. Please use the link below to access the required form to request the funds.

REQUEST SCHOLARSHIP FUNDS

The e-form will ask you for the following information:

• Your name
• Your email address
• The name of the scholarship(s) you were awarded
• Your college, university or technical institute student ID number
• The name of the college, university or technical institute attend9ng in the fall of 2021
• The contact person at the financial aid office the check should be mailed to
• Mailing address of the financial aid office

REQUEST SCHOLARSHP FUNDS

If you received a scholarship from Savage American Legion, Burnsville Athletic Club, Fairview Ridges Medical Staff or Walser Foundation, you do not need to complete the form. Those organizations will contact you directly regarding your scholarship award funds.

**Scholarship funds will be forfeited if not requested by January 24, 2027.
